How it Works

The Navigate Maternity (NM) Kit is provided with a prescription from your healthcare provider. It includes an FDA-approved, Bluetooth-compatible blood pressure cuff and digital scale, allowing you to monitor your clinical numbers and keep them within a healthy range.

With the NM Kit, you'll have access to our app, which offers health screenings and ensures effective heart health management. You, your provider, and our navigator can access your information to promptly address any abnormalities.

The NM Kit is designed for use throughout your pregnancy and may also be utilized during the postpartum period, depending on your health and provider's discretion. This comprehensive approach ensures the best possible outcomes for you and your baby.

Eligibility Check

Patient interested in Navigate Maternity self-monitoring program checks eligibility via website; Patient's eligibility is determined by high risk factors and insurance verification

Start Form Completion

If deemed eligible, the patient completes the Navigate Maternity Start Form

Order

Upon completion, the patient downloads the order form and takes it to the physician for completion. The physician then faxes the Navigate Maternity Order Form to the Navigate Maternity Team

Shipment

After receiving the order and verifying insurance, the Navigate Maternity Team promptly ships the BP cuff to the patient

Download

Upon receiving the BP cuff, the patient downloads and logs into the Navigate Maternity app using the provided instructions

Biometric Screening

After creating a profile, Patient uses Navigate Maternity's Bluetooth devices to take and upload biometric information through the app

Track

The patient tracks vitals throughout pregnancy and for up to a year postpartum and discusses screenings with the physician
